To register, please complete all information and return it to Jeanne Hausler, hauslerj@michigan.gov, by November 27, 2017. Please complete a separate form for each registrant. Space is limited to 2 registrants per health department.
Space is limited to the first 25 registrants, so you are encouraged to sign up early. If you register and find out you can’t attend, please call or email to cancel your registration, so your slot at the training can be filled by someone on the waiting list.
There is no cost to participants for this training, except any travel expenses accrued.
